Welcome to the Township of Lakewood Department of Vital Statistics online ordering system. Requests will be processed between Monday through Friday, 9:00 am to 5:00 pm.

In the Township of Lakewood, the Office of Vital Statistics and Registry registers vital events and maintains Birth, Marriage, and Death records starting in 1918.

The Office of Vital Statistics processes orders within 2 business days, excluding weekends and holidays, once all information is available. 

Please note - New Jersey law protects and restricts the release of Vital Records.  Vital Records are not public records and cannot be searched online.

REQUESTING A NON-GENEALOGICAL CERTIFIED BIRTH, DEATH, OR MARRIAGE CERTIFICATE

Birth Certificates 

• The Office of Vital Statistics can issue Non-Genealogical Certified copies of Births occurring within the last 80 years or if the subject is still alive.  

Death Certificates 

• The Office of Vital Statistics can issue Non-Genealogical Certified copies of deaths for individuals that have passed away in the Township of Lakewood, NJ.

Marriage Certificates 

• The Office of Vital Statistics can issue Non-Genealogical Certified copies of marriages that occurred in the Township of Lakewood, NJ.
